Northgate High School Athletics

Physicals - October 24th at 6:00pm in the NHS gym:  Physicals will be offered for NHS student athletes preparing for the upcoming winter season.  

Required Documents:

Please print, fill out and bring with you the required MDUSD athletic activity clearance form to the physical so upon completion of the exam our sports medicine doctor can sign off.

Reminder - To participate in sports at Northgate you must have a physical between July 1st, 2011 and June 30th, 2012.  

Cost of the physical is $50.00, payable to Bronco Boosters.
